0004055: missing/broken/outdated flags in the flags/voting menus

Description

missing dmflags:
sv_noitems (dmflags 2)
sv_nocountendmonst (dmflags2 67108864)

broken dmflag:
"keep teams after a map change" isnt properly toggling sv_keepteams (zadmflags 128)

missing voting flag:
Make newly connected clients wait before calling a vote. The wait time is controlled with the new CVAR 'sv_voteconnectwait'.

outdated voting flag:
sv_limitnumvotes renamed to sv_votecooldown and changed to integer that determines the time to wait before a player can revote. All vote limits and times are double this value.
