Please post a minimal example wad demonstrating the issue. Also, is this happening in 2.1.2 or 3.0?

This happens in 3.0

How you could replicate it with this wad:
1) Go to multiplayer options
2) Start a team deathmatch game
3) Go to team blue and enter in console: "summon poisonpelletcloud"
4) Enter "freeze" in console
5) Switch teams to team red (or spectate and rejoin as team red)
6) Go to the location where you spawned the poisonpelletcloud and enter "freeze" in console
7) Walk into the cloud.

You will see that the green poison effect will trigger but not the damage over time

Tested online and offline with 3.1 and the health was properly affected by poison damage.
