This seems to be caused by A_FireCustomMissile.
1) Start a standard server with the example WAD.
2) Connect a client to the server and join the game.
3) Fire at will.
There are 2 custom weapons in the example WAD: "PlasmaRifle_02" and "RocketLauncher_02".

This was fixed by Dusk, Torr, and I when we handled the grenade bouncing desync.

Marking resolved things as such is fine, but when you mark something as resolved that you didn't fix on your own, please assign it to the one who fixed the problem instead of yourself. Mantis automatically proposes to do the latter, so one has to be a little careful here.
