The former is a problem, the latter I imagine would be a violation of the rules regardless. As for logging in after death I'm not sure atm, but perhaps the option to do so could be controlled by the server to begin with?
Maybe the server could detect(by IP?) if the player has already exhasted their lives and store that until the map ends?

The only way this would work is if you did sv_forcelogintojoin from the start. So the main issue really lies in making logging-in more convenient. |

That is not an appropriate course of action. The end result of that would be having all survival servers force login to join and the community would not like that. |

Well there's no other way besides that. Actually it could be circumvented if someone had multiple accounts too now that I think about it. |
